I have never tried the infamous "vacuum pump" -- seems kind of bizarre -- but apparently they are quite effective.Here is an article which suggests that they work the same way as Viagra does - via the nitric oxide system. Worth a try?

I own one and I don't think it's anywhere near as effective as Viagra. But it is also much better than nothing. The temporary girth enlargement they offer is nice too (it is temporary though, because the next day everything is back down to original size). If one overdoes it, there can be the annoying "doughnut effect" where the penis gets swollen temporarily with lymphatic fluid, just behind the head of the penis. I see penis pumps as a nice peripheral thing to own but by no means a cure for erectile dysfunction. If someone can get a rigid, turgid, 10-on-a-10-scale-of-hardness from a penis pump, he probably didn't have erectile dysfunction to begin with.
